The CPAP Headgear Strap: What You Need to Know

What the headgear strap does, how to get the fit right, and when to replace it.

The headgear strap holds your mask in place through the night. It doesn't get as much attention as the mask itself, but a poorly fitted or worn-out strap is a common, overlooked cause of leaks and discomfort.

Getting the fit right

  • Snug, not tight. The strap should hold the mask securely without pressing hard enough to leave deep marks or cause discomfort — over-tightening a strap to compensate for a leak usually means the mask itself needs adjusting, a different cushion size, or replacing, rather than more tension.
  • Even tension on both sides. Uneven straps can pull the mask off-centre, causing a leak on one side.
  • Check the fit lying down, not just sitting up — the mask sits differently once you're in your actual sleep position.

Signs the strap needs replacing

  • The fabric or velcro has stretched out and no longer holds its adjustment overnight.
  • Visible fraying or the elastic has lost its stretch.
  • You find yourself re-tightening it repeatedly through the night.
